Past medical and surgical history (PMH)
This consists of a summary of the current state of general health as well as the medical and surgical events in the patient’s life. Document operations; current and past medications; allergies and sensitivities; hospitalisations; illnesses – especially those that may have a bearing on the presenting complaint.
Do not always rely on patient to divulge all prior problems (eg they may forget; they may consider previous events unrelated to current problem; or may be unwilling to disclose past events).
