Synovial Plica/Plica Syndrome

Plica is a band of tissue – goes from lateral femoral condyle  around superior and medial aspects of patella  down to fat pad anteriorly. Normally causes no problems  occasionally irritation of medial facet of patellar.

Clinical features:
Pain on anterior and medial side of knee.
Pain and tender to palpation 1cm medial to patella – may be able to palpate tight band
Pain after prolonged standing. Can have snapping sensation.

Treatment:
Activity modification; NSAID’s; stretching of patella retinaculum; strengthening vastus medialis; physical therapy
