Initial management:
Cleansing of wound; tetanus prophylaxis; grossly contaminated wounds debrided and left open; non-weightbearing; close observation for infection.
Prophylactic antibiotics have been shown to be of little value use discretion
If get osteomyelitis if medical management fails surgical debridement of involved bone and parental antibiotics for two weeks
