Cutaneous Pressure Perception:
Semmes-weinstein monofilaments –
5.07/10g monofilament – widely considered as the best indicator of protective sensation.
Can easily under or over report neuropathy if not used properly.
Technique:
• patient supine; instructed to say ‘yes’ when they feel the monofilament
• monofilament is applied with enough force for it to buckle and held in place for one second
• generally applied to 10 different sites on each foot – plantar to the first, third and fifth digits; plantar to the first, third and fifth metatarsal heads; medial and lateral plantar aspect of the midfoot; plantar heel; and dorsal midfoot
After several uses the monofilament buckles at less force – needs time to recover
